Keramas itself is one of Bali's marquee reef breaks, a fast, hollow, barreling right-hander over shallow volcanic rock that needs a rising-to-high tide to cover the reef; it is strictly advanced/expert terrain and gets heavily crowded with locals and touring pros. The surrounding black-sand stretch (Ketewel, Saba, Lembeng, Pabean) is quieter and gentler, beach/river-mouth peaks better suited to improvers and casual paddling, per on-the-ground guides. Best season is wet season (roughly November-March/April) when SE-S swell and favourable N-W winds line up; Keramas specifically wants at least an hour past low tide to keep the sharp reef covered.
